Community Health Programs

MDHA Community Health Workers (CHW) manage programs and services that provide health and wellness resources to residents. They also assist MDHA residents with accessing health-related services and appointments.

Resources for Pregnant Mothers and Caregivers

Transportation Services

CHWs coordinate free shuttle rides to local grocery stores, retail stores and pharmacies for MDHA residents. Residents are responsible for purchasing their own items at these stores. Accessible shuttles are available upon request. To learn more, contact the MDHA Resident Services Information Line.
